Hospital Bridge Program - Sumter County

The Hospital Bridge Program in Sumter County provides immediate peer recovery support to individuals experiencing substance use challenges during or following a hospital visit. Our trained Certified Recovery Peer Specialists work alongside hospital staff to connect patients with resources, provide emotional support, and assist with developing personalized recovery plans.

Our goal is to ensure no one leaves the hospital without knowing they have options, support, and a path forward. Whether it's navigating treatment, accessing community services, or simply having someone who understands, the Hospital Bridge Program offers compassionate, nonjudgmental support during a critical time.
